Job summary

Traditional Home Health Care in Pennsylvania is seeking an LPN for hospice services. Nurses will visit patients in their homes across Tunkhannock, Montrose, Clarks Summit and nearby areas. Office is in Dunmore, PA; mileage and travel are part of the role.

You will assess patients, follow the care plan with the physician and nursing supervisor, document thoroughly, and communicate with patients and families. Home-health experience preferred.

Qualifications

• Current license as a Practical Nurse (LPN) in Pennsylvania.
• Excellent assessment skills, documentation skills and ability to meet deadlines.
• Ability to travel to case assignments.
• Ability to work independently with minimal supervision and manage time effectively.
• Strong interpersonal and verbal/written communication skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assesses patient status and follows plan of care in collaboration with physician, nursing supervisor, patient and family members.
• Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with physician orders.
• Reports patient status changes and revises plan of care in consultation with physician.
• Maintains meticulous documentation of interactions, medications, and plan changes.
• Acts as liaison with patients, therapist, nursing supervisor, and other health care team members.
• Informs patients of other services and makes referrals as needed.

Job responsibilities include, but not limited to the following:

• Assesses patient status and follows plan of care in collaboratedwithphysician, nursing supervisor, patient and family members.
• Administers patient care, management and evaluation of the care plan, teaching of the patient in accordance with physician orders.
• Reports patient status changes, care, and progress to physician and nursing supervisor on a continuous basis. Revises plan of care by consultation with physician.
• Timely and accurately prepares, maintains and submits meticulous documentation as required by Traditional Home Health Care including all interactions with patients, case conferences, medication changes,time points, and changes to plan of care.
• Acts as a liaison and maintains contact with patients, therapist, nursing supervisor, and other members of the health care team.
• Informs patients of other services that are available and makes referrals to other community resources as needed.

Requirements:

• Current license as a Practical Nurse (LPN) in Pennsylvania
• Must have current driver's license
• Ability to travel to case assignments
• Excellent assessment skills, documentation skills and ability to meet deadlines
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal skills.
• Demonstrates good verbal and written communication, and organizational skills
• Acute care and/or home health experience is preferred.
