Job summary

JourneyCare Hospice in Carol Stream, IL is seeking a registered nurse to serve as the hospice RN Case Manager. This full-time role offers a Monday–Friday schedule with a limited weekend rotation and a dedicated caseload focused on comfort and dignity.

You will perform patient assessments, admissions, and individualized care planning, coordinate with physicians and the interdisciplinary team, provide end-of-life support, and maintain timely documentation.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments admissions and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed; provide end of life support including family education and pronouncement.
• Maintain timely accurate documentation.
• Supervise Home Health Aides in compliance with regulations.
