JourneyCare Hospice in Carol Stream, IL is seeking a full-time Hospice Registered Nurse (RN) Case Manager to coordinate end-of-life care with an interdisciplinary team. You will manage a dedicated caseload, perform patient assessments, admissions, and care planning, delivering skilled nursing care focused on comfort and dignity.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and proof of insurance.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed; provide end-of-life support including family education and pronouncement.
