Moments Hospice is seeking an experienced RN Case Manager to lead clinical care for patients and families in hospice settings. You will manage symptoms, coordinate services, and provide guidance through end-of-life care, with a supportive leadership structure and reasonable caseloads.

Qualifications

• Active RN license in the state you work in (WI).
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
