Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical care for patients and families in the hospice setting. You will assess, plan, and coordinate comprehensive services to relieve symptoms and support end-of-life goals.

You will collaborate with a multidisciplinary team, educate families, and navigate care options while maintaining compassionate, patient- and family-centered care. The role is field-based with a strong leadership component and opportunities for professional growth.

Qualifications

• Active RN license in the state you work.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
