Job summary

Your Health Organization is seeking a Registered Nurse as a Nurse Case Manager to serve Beaufort/Hardeeville and surrounding areas. You will visit patients in homes and facilities, travel daily, and coordinate with providers to initiate telemedicine visits and care plans.

This full-time role requires flexibility and travel across service areas including Beaufort, Hardeeville, and Port Royal. We focus on quality patient care across facilities, homes, clinics, and virtual visits.

Qualifications

Must be a Registered nurse.
Three (3) years clinical experience preferred.
Experience in community settings preferred.
Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
Must be a licensed driver with reliable transportation.

Responsibilities

Facilitate receiving all medical records from the patient’s primary provider and specialists.
Review medical records and enroll patients in Care Management.
Initiate a Care Management Plan of Care for eligible patients.
Coordinate with the patient’s health care team, providers, therapists, and other care representatives.
Facilitate telehealth visits and coordinate care with appropriate providers.
Reconcile medications and assess home health, hospice, or RPM options.
Document Care Management activities and keep plans up to date.
