AMEDISYS HOME HEALTH is hiring a registered nurse case manager in Winston-Salem, NC for a full-time role with day shifts and weekend coverage. Compensation ranges from $74,000 to $92,000 per year, plus a $10,000 sign-on bonus.

Qualifications

• 1+ year of clinical RN experience; if less, approval by VP Clinical.
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note: if <6 months RN experience, participate in RN Intern program.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
