Amedisys Home Health is seeking a Registered Nurse Case Manager for a full-time day shift. You will evaluate patients, coordinate with the care team, and implement individualized plans of care to promote healing at home.

Qualifications include 1+ year RN experience, active RN license, CPR, and a valid driver’s license. Competitive pay with a $20,000 sign-on and comprehensive benefits are offered.

Qualifications

• 1+ year of RN clinical experience (or as approved by VP Clinical).
• Current RN license in applicable state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
