Job summary

OneWell Health Care is seeking an RN Case Manager in Savannah, GA to provide clinical leadership and case management for home health services. The role requires assessing client needs, coordinating services, and ensuring regulatory compliance.

The ideal candidate holds a GA RN license, a BSN, and has at least 2 years of nursing experience with case management or IDD/waiver exposure. This is a full-time, on-site position with opportunities for growth and staff development.

Qualifications

• Active RN license in Georgia is required.
• BSN is required; MSN preferred.
• Minimum of 2 years nursing experience; case management experience preferred.

Responsibilities

• Coordinate and oversee care for assigned clients to align with care plans.
• Perform nursing assessments and develop/monitor individualized care plans.
• Ensure compliance with regulatory, payer, and organizational requirements.
• Participate in quality improvement initiatives, audits, and action plans.
• Collaborate with caregivers, providers, and community resources for continuity of care.
• Support training and onboarding of nursing staff and supervise field employees.
