AccentCare, Inc. is seeking an RN Case Manager for home health in Georgia. The role manages patient care plans from admission to discharge, coordinating with nurses, LPNs, and CHHAs to deliver the plan of care.

The ideal candidate will be a graduate of an approved nursing program, hold a current Georgia RN license, have at least one year of RN experience, and CPR certification. Schedule is Monday–Friday with on-call responsibilities.

Formación

• Graduate from an approved nursing program and licensed to practice as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsabilidades

• Assess nursing needs and manage patient care plans from admission to discharge.
• Complete initial patient assessment with history at admission.
• Identify appropriate nursing diagnoses.
• Review medications for safety and interactions.
• Set measurable goals and communicate interventions to the physician.
• Implement nursing interventions and re-evaluate needs as required.
• Document assessments, progress, and discharge notes promptly.
• Inform the physician and team of changes in patient condition.
