Wellstar Health System is seeking an RN Case Manager to provide and supervise primary care for 25-35 patients in their homes, coordinating care across RNs, LPNs, therapists and social workers.

You will develop and implement plans of care, adjust visit frequency per physician orders, and ensure timely documentation in the EMR while maintaining high-quality patient outcomes. Strong critical care and leadership skills are essential.

Competenze

• Minimum 1 year of Medical/Surgical Nursing experience; home health and ICU preferred.
• Strong critical care assessment and clinical skills.
• Excellent communication and leadership abilities.

Mansioni

• Assess and coordinate patient care across disciplines.
• Supervise LPNs and Home Health Aides.
• Manage patient plans from admission to discharge with physician orders.
• Document patient status and plan of care in EMR within required timeframes.
• Educate patients and caregivers to promote self-care and safety.
