Job summary

CenterWell Home Health in Savannah, GA seeks a Home Health Registered Nurse to deliver personalized, in-home care. Reporting to the Clinical Manager, you will develop and manage care plans, administer medications, and support patient independence in the comfort of their own homes.

You will collaborate with physicians and clinicians, educate families, supervise Home Health Aides, and document care activities.

Qualifications

• Med surg, ICU, ER, or acute care experience.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care at patients' homes, including meds, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatments.
• Maintain accurate records and coordinate with healthcare team.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure correct documentation.
