Job summary

Bridge Home Health and Hospice is seeking a registered nurse to serve as a Case Manager in Sonoma County, CA. This full-time role directs home care services, plans care with the patient and family, and coordinates with physicians to deliver nursing care in patients’ homes.

The ideal candidate has a California RN license, CPR certification, and at least two years of licensed home health experience, with a bachelor’s degree preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience in California.
• Current licensure in state and CPR certification.
• Bachelor’s degree with one year of home health care preferred.
• Must possess a valid California Driver’s License.
• Ability to lift and assist patients as required.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes revisions as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes family in the planning.
• Administers medications and treatments as prescribed by the physician; provides direct patient care.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions per assessment and plan of care.
• Coordinates discharge planning as part of the care plan.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Participates in on-call duties and ensures equipment and items are available.
