Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience (California).
• Current licensure in state and CPR certification.
• Bachelor's degree, with one year of home health care experience preferred.
• Must possess a valid California Driver's License.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes revisions as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed.
• Provides patient and family counseling.
• Provides health care instructions according to the plan of care.
• Identifies discharge planning needs before discharge.
• Acts as Case Manager and coordinates patient care for the caseload.
