Bridge Home Health East Bay in Petaluma, CA is seeking a Full Time Registered Nurse to plan, organize and direct home care services, bringing community health experience to patients and families in their homes.

The role requires California RN licensure, CPR, and at least 2 years licensed nursing experience in California; BSN preferred. You will assess patients, develop care plans, supervise aides, and coordinate with physicians to ensure high-quality care.

Qualifications

Graduate of an accredited school of nursing.
Minimum of 2 years licensed nursing experience in California.
Current licensure in state and CPR certification.
Bachelor’s degree, with one year of home health care experience preferred.

Responsibilities

Completes an initial patient and family assessment to determine home care needs.
Regularly re-evaluates patient nursing needs and revises the plan of care.
Initiates and revises the plan of care as patient status changes.
Uses health data to determine nursing diagnoses and develop goals.
Administers medications and treatments as prescribed; provides direct patient care.
Counsels patients and families on nursing and related needs.
Provides health instructions per assessment and plan of care.
Identifies discharge planning needs and coordinates prior to discharge.
Acts as Case Manager and coordinates patient care for caseload.
