Qualifications

Graduate of an accredited school of nursing.
Minimum of 2 years licensed experience (California).
Current licensure in state and CPR certification.
Bachelor’s degree, with one year of home health care experience preferred.
Management experience not required; supervise home health aides.
Excellent observation, verbal and written communication skills.
Able to lift and transfer patients; drive a vehicle if required.

Responsibilities

Completes an initial patient/family assessment to determine home care needs.
Develops and revises the care plan with patient and family involvement.
Administers treatments and medications as prescribed.
Counsels patient and family on nursing and related needs.
Communicates with physicians and other health professionals every 60 days.
Coordinates patient care for assigned caseload; acts as Case Manager when needed.
On-call duties and ensuring equipment and services are available.
Supervises home health aides and ensures attendance and reliability.
