Amedisys is hiring a registered nurse case manager for full-time day visits across Daleville, Fincastle, Eagle Rock, Bedford Montvale and parts of Roanoke. As part of the role, you will assess patients, collaborate with the care team to implement plans, supervise LPNs and HHAs, and ensure timely documentation while keeping the clinical team

Qualifications

• One (1+) year of clinical experience as an RN (more is preferred).
• Current RN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note: if <6 months RN experience, RN Intern program participation may be required.

Responsibilities

• Performs patient assessments and collaborates with care team to develop a plan of care.
• Refers to other disciplines as indicated and documents rationale.
• Promotes health and independence through teaching and rehab measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to clinical manager and care team.
• Plans and provides staff education.
• On-call responsibilities for patients/families as assigned.
• Participates in clinical development and continuing education.
• Other duties as assigned.
