Job summary

Amedisys in Massachusetts invites you to join as a registered nurse case manager. You will perform patient assessments, collaborate with the care team to develop and implement a plan of care, and coordinate referrals as needed to meet each patient’s goals.

This full-time daytime role covers North Attleboro, Norton, Mansfield, Plainville and surrounding areas. You will supervise LPNs and HHAs, document care promptly, and participate in ongoing clinical education to support high-quality home

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN).
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• RN Intern program eligibility if less than 6 months RN experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
