Amedisys is seeking a Registered Nurse Case Manager for full-time daytime shifts. You will assess patients, coordinate with the care team, and promote independence through teaching and rehabilitation at patients' homes. The role includes supervising LPNs and HHAs, timely documentation, and on-call coverage as needed.

Qualifications

• 1+ year of clinical RN experience required.
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated.
• Promotes patient health and independence through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in ongoing clinical development and education.
