Job summary

Amedisys Home Health is seeking an experienced RN Case Manager to assess patients, develop and implement care plans, and coordinate with the care team to support healing at home. This role supervises LPNs and HHAs and requires timely documentation and on-call coverage.

Coverage includes Daleville, Fincastle, Eagle Rock, Bedford, Montvale and part of Roanoke with pay between $74,000 and $92,000 annually, plus comprehensive benefits and retirement options.

Qualifications

• One (1+) year of clinical RN experience; if less, VP Clinical approval required.
• Current RN license for assigned state(s).
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
