Amedisys is seeking a registered nurse case manager to perform patient assessments and coordinate home health services. The role includes developing care plans, supervising LPNs and HHAs, and communicating progress to the care team as part of a comprehensive patient care program.

Qualifications

• 1+ year of clinical RN experience or approval by VP Clinical.
• Current RN license for assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
