Job summary

AccentCare Inc. in Exton, PA is seeking a Patient Care Manager to lead the home health nursing team and ensure high-quality patient care in an in-office setting.

This role focuses on clinical supervision, coordinating patient care, staff development, and compliance with regulations, while fostering a culture of safety, professionalism and service. The position is full-time, Monday–Friday, 8am–5pm, with salary between $90,000 and $105,000 per year, based on experience.

Qualifications

• RN licensed and eligible to practice in the state
• Two years of supervisory or management experience preferred
• Knowledge of Medicare Conditions of Participation and regulatory requirements

Responsibilities

• Provide clinical supervision to ensure patient care aligns with professional standards and regulations
• Coordinate and monitor patient care across the team
• Supervise clinical personnel to ensure quality and consistency of services
• Maintain effective communication with patients, caregivers, referral sources, and staff
• Support quality outcomes and participate in quality improvement initiatives
• Uphold safety and professional standards and support staff development
