Job summary

IntelliResume is seeking a Registered Nurse Case Manager located in Milford, Connecticut. This role involves admitting patients for home care, developing patient care plans, and ensuring the safe delivery of services.

Candidates should hold a nursing diploma or Associates degree and preferably a Bachelor's in Nursing, with at least two years of acute care nursing experience. A valid Connecticut RN license and CPR certification are required, along with strong clinical and organizational skills.

Qualifications

• Minimum two years of nursing experience in an acute care setting.
• Previous home care experience is desirable but not required.
• Registered Nurse with a valid Connecticut license required.

Responsibilities

• Assesses/reassesses patients and establishes plans of care.
• Collaborates with providers and communicates all changes promptly.
• Oversees LPNs and Home Health Aides.
