Job summary

FreedomCare is seeking a Registered Nurse (RN) Home Health Case Manager in Middlebury, Connecticut. The role focuses on in-home assessments, care coordination, education for patients and caregivers, and ongoing care management to ensure safe and effective services.

The position is a staff, 40-hour per week role with dayshift hours. New graduates are welcome; travel within CT is required, and a valid RN license in Connecticut is required.

Qualifications

• Diploma from an accredited nursing program.
• Active RN license in Connecticut.
• New graduates welcome; home care case management a plus.
• Willingness to travel within assigned geographic area; valid driver’s license.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation.
• Develops and manages patient’s plan of care.
• Coordinates care with physicians and other disciplines; documents changes.
• Provides caregiver education and connects families to resources.
• Ensures regulatory compliance and ongoing quality assurance.
