Unifi Healthcare is seeking a registered nurse Case Manager to develop, implement and evaluate individualized care plans in the home setting. You will serve as the liaison between the agency, patients and families, ensuring quality care and adherence to policy and procedure.

Responsibilities include documenting and reviewing plans, coordinating with authorization specialists, educating families, and supervising clinical activities within your service area.

Competenze

• Current RN licensure.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience.
• Ability to work independently, multi-task, prioritize, and function in a team environment.
• Professional, courteous, compassionate and customer-oriented demeanor.

Mansioni

• Performs documentation/chart review to ensure accuracy and that the client’s POC is being followed.
• Maintains accurate information in QuickBase for all assigned clients.
• Manages care implementation with attention to quality patient care and patient family satisfaction.
• Coordinates care authorization with Authorization Specialist(s) to establish the foundation for the client’s treatment plan of care.
• Provides education to client regarding benefits and coordinates with team members to accomplish objectives.
• Maintains ongoing responsibility for assigned caseload and attends pertinent appointments and meetings.
• Communicates pertinent information with DON/ADONs, Operations, Authorizations, Billing.
• Provides clinical guidance and coaching to staff and patient families.
