Job summary

UnitedHealth Group is seeking an RN Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge planning. Under supervision of an RN or MD, you will educate, coach, and assist with treatment decisions while collaborating with Care Advocates and Social Workers.

Primary responsibilities include patient assessments, developing care plans, supervising staff, on-call duties, and ongoing education to enhance care outcomes.

Qualifications

• Current RN license in the state(s) assigned.
• Current CPR certification.
• 1+ year(s) of clinical RN experience.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by patient needs.
• Promote patient health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Participate in on-call responsibilities and services.
