Job summary

Team Select Home Care in Houston, TX is seeking an experienced registered nurse Case Manager to oversee patient care per physician orders and the plan of care. You will coordinate with physicians, families, and staff to deliver patient-centered care and ensure compliance with requirements.

Qualifications include a nursing license, at least 1 year of nursing experience (home health preferred), and current CPR. Strong communication, documentation, and time management are essential.

Qualifications

• Average computer skills with competency in media communications.
• Excellent verbal and written communication.
• Must maintain licensure and continuing education; current CPR is required.

Responsibilities

• Develops and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management for patients and ancillary staff.
• Performs initial evaluation and regular reassessments per state/payor requirements.
• Coordinates care between patients, families and providers; facilitates referrals as needed.
• Documents nursing activities, medication reconciliation, and care notes; participates in case conferences and on-call duties.
