Job summary

Team Select Home Care in Denver, CO is seeking an RN Case Manager Support to oversee skilled nursing care in patients’ homes under physician orders and plans of care. The role blends hands-on nursing with intermittent case management to deliver high-quality home health services.

The RN must hold an active license, have at least 1 year of nursing experience (home health preferred), and be able to coordinate care with patients, families, and providers, following state and federal requirements.

Qualifications

• RN license in applicable state(s) and current CPR certification.
• Graduate of an approved school of professional nursing.
• At least 1 year of nursing experience; home health experience preferred.
• Strong communication, documentation, and clinical assessment skills.
• Ability to manage care plans and coordinate with families and providers.

Responsibilities

• Develop and evaluate individualized patient-centered care plans.
• Provide case management for assigned patients and coordinating staff.
• Perform initial and ongoing nursing assessments and document data per regulations.
• Communicate changes in patient condition to physicians and caregivers.
• Educate patients and families on home health strategies and resources.
• Administer medications per physician orders and agency policy.
• Participate in case conferences and ensure quality of care per policies.
• Maintain accurate records and confidentiality; submit data timely.
