Job summary

The Bridge Home Health and Hospice is seeking a registered nurse to plan, organize and direct home care services in Solano and Sacramento counties. The ideal candidate has strong community health experience and patient-centered care in home settings.

You will assess patients, develop care plans, coordinate with physicians, and educate families while delivering quality nursing care in patients' homes. California RN license and CPR are required.

Qualifications

• Graduate of an accredited nursing program; California RN license with CPR.
• Minimum 2 years of licensed nursing experience in California.
• Bachelor’s degree with 1 year home health experience preferred; supervisory not required.

Responsibilities

• Completes an initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments per physician orders; provides direct patient care.
