• Collaborate with physicians, therapists, and social workers to coordinate care.
• Maintain accurate documentation: progress notes, care plans, med records.
• Provide education on health management and safety practices.
• Participate in case conferences and team meetings to improve care.
• Act as liaison between clients, caregivers, and health professionals.
• Coordinate and oversee implementation of care plans with other professionals.
• Provide ongoing monitoring and adjust plans as needed.
