ComForCare Home Health Care - Arlington VA is seeking an experienced RN Case Manager (Assessment Only) to perform in-home assessments and coordinate patient care. This role emphasizes autonomy, clinical excellence, and compassionate care, with opportunities for advancement and flexible scheduling.

You'll complete OASIS and start-of-care assessments, collaborate with physicians and families, and ensure compliant, timely documentation in an EHR system.

Qualifications

• Active and unrestricted RN license in Virginia is required.
• 1–2 years of home health or clinical nursing experience preferred.
• Proficiency with OASIS documentation and Medicare home health regulations.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, time management, and organizational abilities.
• Reliable transportation and valid driver's license for home visits.
• Comfortable working independently in a field-based, patient-facing environment.

Responsibilities

• Conduct comprehensive in-home patient assessments to determine care needs and eligibility for home health services.
• Complete OASIS assessments, start-of-care evaluations, and recertification assessments in compliance with regulatory standards.
• Collaborate with physicians, care teams, and family members to develop and communicate individualized care plans.
• Document clinical findings accurately and in a timely manner using electronic health records (EHR).
• Ensure compliance with Medicare, Medicaid, and Joint Commission guidelines during all assessment activities.
• Communicate patient status changes and clinical concerns to the appropriate care team members.
• Maintain a flexible schedule to accommodate patient and agency assessment needs across the Arlington, VA service area.
