• Oversee caregivers' adherence to care plans through regular visits.
• Ensure compliance with state regulations and agency policies.
• Collaborate with healthcare team members for coordinated care.
• Maintain accurate documentation of client care.
• Provide education and support to clients and families.
• Participate in case conferences and team meetings.
• Act as a liaison between clients, caregivers, and healthcare professionals.
• Coordinate care plan implementation with other professionals.
• Monitor and evaluate client progress, adjusting care plans as necessary.
