Job summary

A community-based non-profit home health provider is seeking a dedicated Registered Nurse (RN) Case Manager to deliver compassionate, patient-centered care. In this role, you will coordinate care for patients in their homes, educate families on management strategies, and ensure timely documentation. Qualifications

• Minimum 1 year of nursing experience (home health or hospice preferred).
• Medicare/OASIS documentation experience a plus.
• Current CPR/BLS certification (or ability to obtain within 30 days of hire).
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care and support to patients in their homes.
• Coordinate care with physicians and other team members.
• Complete OASIS documentation and monitor patient conditions.
• Educate patients on disease management and medications.
