Job summary

Transitions LifeCare seeks to hire full-time Community-based Hospice RN Case Managers who provide compassionate, expert care to patients and families as they navigate end-of-life and serious illness.

The role includes a $20,000 sign-on bonus for eligible experienced RNs, a Monday–Friday schedule with occasional weekend or holiday coverage, and travel to patient homes within designated counties in North Carolina.

Qualifications

• Must have current NC RN license and be able to travel to patient homes.
• BSN preferred but not required.
• Must pass pre-employment tests and annual trainings.

Responsibilities

• Plan, provide, supervise and document hospice nursing care for patients.
• Collaborate with IDG Team and other internal/external partners to achieve patient care goals.
• Use nursing judgement to individualize the plan of care based on assessment of each patient.
• Maintain comfort, privacy, and dignity of patients.
• Develop working knowledge of regulations, policies, and procedures; ensure compliance.

Transitions LifeCare seeks to hire full-time Community-based Hospice RN Case Managers who embrace our mission of caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness.

Job Details

This is an exciting time to join our team! Qualifying Experienced RNs with at least 1+ years of experience may be eligible for up to a $20,000 Sign-On Bonus. This bonus is available for select positions with variable total amount dependent upon priority need, job territory, and other qualifying factors. Details and Conditions apply.

The schedule for this position is Monday-Friday 8:00am-5:00pm; available one weekend shift per quarter; one major or two minor holidays per year.

• Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance

Requirements

• Registered Nurse with current NC license; BSN preferred.
• Must have reliable transportation to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
• Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
