Transitions LifeCare is hiring a Full-time CNA to provide compassionate care in the Raleigh, NC area. The role supports patients under an individualized care plan and works with an interdisciplinary team to meet care goals.

The position offers a Monday–Friday schedule with rotating weekends/holidays and benefits including a $1,000 sign-on bonus, six weeks of PTO for 40-hour employees, and mileage reimbursement.

Qualifications

• Current CNA listing with NC Home Health Aide Registry.
• Must have reliable transportation.
• Hospice or home health experience preferred.
• Must pass all applicable pre-employment and annual tests and meet guidelines.

Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness.

The schedule for this position is Monday-Friday 7:00am-3:30pm; rotating weekends and holidays.
