Job summary

Vibra Healthcare is seeking an RN Case Manager to oversee the integration of case management into patient care, discharge planning, and home care coordination. You will work with hospital staff to ensure smooth transitions and optimal patient outcomes.

The role emphasizes psychosocial assessments, care planning, and collaboration with physicians and nurses to design effective patient care plans. A BSN or related degree and current RN/SW licensure are required.

Qualifications

• Bachelor's degree in healthcare or social services; RN or SW preferred.
• Licensed RN or SW in state of employment; BLS within 30 days.

Responsibilities

• Directs integration of case management into patient care, discharge, and home planning.
• Coordinate clinical care with patients, families, and hospital staff.
• Perform psychosocial assessments and develop care plans with multidisciplinary teams.
• Arrange discharge and post-hospital care with community services.
• Provide guidance on post-discharge treatment and recovery options.
