Job summary

Heart to Heart Hospice in Indiana is seeking an RN Case Manager to coordinate hospice nursing care for patients with life-limiting illnesses. You will perform comprehensive assessments, develop and revise the individualized plan of care, collaborate with IDG members, and ensure regulatory compliance.

Ideal candidates will hold a nursing degree and an active RN license, have reliable transportation, and be willing to participate in on-call duties as assigned.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of experience as an RN preferred in a healthcare/clinical setting.
• Current Driver's License and reliable transportation with auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized plan of care and revise as necessary.
• Collaborates care amongst patient, caregiver, physicians and healthcare professionals.
• Attend IDG meetings, coordinate patient care planning, and promote coordination of care.
• Perform on-call responsibilities and provide on-call service to patients and families as assigned.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
