Job summary

Heart to Heart Hospice is seeking an RN Case Manager to perform comprehensive patient assessments and implement individualized care plans. You will collaborate with physicians and the care team to coordinate services and attend IDG meetings to prioritize patient needs.

The role requires RN licensure, a valid driver’s license, and reliability for on-call coverage; prior nursing experience is preferred. This is a full-time position with strong benefits and opportunities to make a meaningful impact.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and recommend revisions to the plan as necessary.
• Collaborates care among patient, patient caregiver, physicians and healthcare professionals.
• Attend IDG meetings and coordinate the patient care planning process with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation.
• Perform other duties as required.

• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
