Job summary

Heart to Heart Hospice is seeking an experienced RN Case Manager to lead comprehensive patient assessments and collaborate with physicians and the care team to manage terminal illness needs.

You will develop and adjust care plans, participate in IDG meetings, ensure timely documentation, and provide on-call coverage as needed in a full-time capacity. Requires active RN license and 2 years of healthcare experience, with a valid driver's license and reliable vehicle.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation, and provide ongoing valid auto liability insurance.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend IDG meetings, coordinate and participate in patient care planning and prioritization.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.
