Heart to Heart Hospice seeks an experienced RN Case Manager to lead comprehensive assessments and coordinate care for patients with terminal illness. You will implement a personalized plan of care, collaborate with a multidisciplinary team, and participate in IDG meetings to ensure timely, compliant care planning.

The role requires an RN license, valid driver's license, and 2+ years of nursing experience (preferably in clinical settings).

Formación

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance.
• Two years of experience as an RN (preferred in healthcare/clinical setting).

Responsabilidades

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs, including risk for grief, culture and spirituality, verbal and non-verbal cues.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient, caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting and coordinate patient care planning with IDG.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies.

Educación

Nursing diploma/degree (Diploma/Associate/Baccalaureate)
