Job summary

CVS Health is seeking a Registered Nurse Case Manager in New York, NY. This field-based role conducts member assessments, annual reassessments, and change-in-condition visits for NY Better Health enrollees. Travel up to 75% and a typical full-time schedule with daytime hours are involved.

The ideal candidate holds an Associate's degree in Nursing (Bachelor's preferred) and an unrestricted NY RN license, with strong clinical assessment and care coordination abilities.

Qualifications

• Associate's degree in Nursing required (Bachelor's preferred).
• Current, unrestricted RN license in NY.
• Strong clinical assessment and care coordination skills.

Responsibilities

• Conduct member assessments for NY Better Health applicants.
• Perform annual, semiannual, and change-in-condition assessments.
• Submit complete documentation within 24 hours of each visit.
• Communicate findings to interdisciplinary teams for care planning.
• Maintain punctuality for 2-3 daily home visits.
• Operate in a remote/telephonic environment with cross-functional partners.
