Munson Healthcare in Michigan seeks a Hospital-based Case Manager to coordinate patient care, ensuring continuity with compassion and reliability. You will work with physicians to evaluate, educate, and refer patients for the next level of care, while coordinating placement and a safe discharge.
Required active Registered Nurse licensure in Michigan and at least 3 years of progressive clinical experience (including 2 years as an RN); Case Management Certification is preferred and may be obtained

A Day inthe Life
A Hospital based Case Manager coordinates and manages patient care to ensure continuity of care delivered with compassion, excellence, and reliability. The Case Manager collaborates with the physicians to evaluate, prepare, educate, and refer patients for the next level of care and works closely with ancillary services to assist in arranging placement in the appropriate care setting as well as coordinates a safe and timely discharge plan.
What’s Required
Full-Time: 40 Hours one week, 32 Hours the next week -- Monday to Friday: 8:30 AM to 5:00 PM
Approximately three on-call weekends per year
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
