Job summary

Qualifications

• 3 years Healthcare/Medical-Acute Care required.
• Must be able to work with acutely & chronically ill patients and their families.
• Excellent interpersonal communication, multi-tasking, prioritizing & organizational skills.

Responsibilities

• Assess and document clinical, psychosocial and financial needs of patients, including care support and discharge considerations.
• Apply InterQual to determine level of service and intensity of care; collaborate with physicians to resolve conflicts.
• Identify plan of care, estimated length of stay and transition/discharge plan with patients and families.
• Coordinate with nursing and social work to ensure plan completion and avoid gaps or duplicative services.
• Discharge/transition patients timely and document variances from plan or target dates.
• Escalate concerns and barriers to appropriate treatment or transition as needed.
• Maintain knowledge of facilities and resources for patients and caregivers.
