Responsibilities

• Participate in unit-based IDR morning and afternoon huddles.
• Coordinate referrals for DME, HH, and hospice.
• Utilize predictive analytic software (e.g., JVION).
• Complete face-to-face patient assessments and coordinate care transitions.
