Qualifications

• Registered Nurse with a Bachelor's degree required.
• Three years of recent clinical health care experience preferred.
• Current Virginia RN licensure and BLS within 6 months.

Responsibilities

• Provide case management for assigned patient populations to optimize outcomes.
• Perform needs assessments, facilitate referrals, and coordinate post-acute care.
• Collaborate with Utilization Review and Social Work to ensure smooth transitions.
• Maintain documentation reflecting condition, comorbidities, and plan of care.
• Participate in interdisciplinary meetings and utilize predictive analytics where applicable.
