Job summary

Carilion Clinic in Roanoke, VA is seeking an RN Case Manager for the ED Observation Unit. You will provide organized case management, coordinate referrals, and support discharge planning across the health care continuum.

Applicants should have a Bachelor’s in Nursing or equivalent hospital experience, Virginia RN licensure, and at least three years in clinical care with discharge planning responsibilities. This position offers a weekend, full-time schedule and competitive benefits.

Qualifications

• Registered Nurse with a bachelor's degree or equivalent hospital experience.
• Three years of clinical health care experience with care planning and discharge planning.
• Current Virginia RN licensure and BLS-HCP within 6 months of hire.

Responsibilities

• Provide case management for assigned patient populations.
• Promote cost-effective care and coordinate referrals across the health care continuum.
• Perform needs assessments and facilitates post-acute care linkages.
• Collaborate with Utilization Review and social work to optimize discharge planning.
• Document accurately and ensure compliance with CMS and hospital policies.
• Participate in Interdisciplinary Team Meetings and performance initiatives.
