Qualifications

• RN with Virginia licensure; Bachelor's degree preferred or five years RN hospital experience in lieu of degree.
• Three years of RN hospital experience with discharge planning and utilization management; supervisory experience preferred.
• Current AHA BLS–HCP within 6 months of hire.

Responsibilities

• Collaborates with Utilization Review to support patient status and care planning.
• Maintain contact with UR nurse and social work; document progress and referrals.
• Coordinate post‑acute referrals (home health, hospice, DME) and ensure smooth transitions.
• Advocate for patients and families throughout the episode of care.
