Job summary

Qualifications

• Registered Nurse license in Virginia; Bachelor’s degree preferred or RN experience in lieu of degree.
• Three years of recent clinical health care experience with care coordination responsibilities.
• Current AHA BLS‑HCP certification within 6 months of hire.
• Strong communication, problem-solving, and interdisciplinary teamwork.

Responsibilities

• Collaborates with Utilization Review Nurse to coordinate patient care and discharge planning.
• Maintains regular contact with UR Nurse and Social Work throughout the day for status updates.
• Uses InterQual information to support medical necessity and accurate admission status.
• Documents patient condition, treatments, and plan of care promptly in the medical record.
• Coordinates referrals to post‑acute services including home health and hospice.
