Job summary

Community Hospital is hiring a Full Time Registered Nurse Case Manager. This role focuses on comprehensive coordination of care, resource utilization, and leading the multidisciplinary team to achieve optimal patient outcomes.

Strong communication and teamwork are essential as you guide care from admission through discharge. The ideal candidate will hold Oklahoma RN licensure, with BSN preferred and at least three years of hospital-based nursing and utilization/case management experience.

Qualifications

• Current licensure with the Oklahoma State Board of Nursing.
• BSN preferred but not required.
• Minimum of three years hospital-based nursing practice with experience in utilization/case management.
• BLS for Healthcare Providers required within 30 days of hire.

Responsibilities

• Coordinate care and services for individual patients and patient populations.
• Promote efficient use of resources and care pathways.
• Lead or collaborate with a multidisciplinary team to achieve outcomes and physician satisfaction.
• Meet patient needs from admission through discharge and support ongoing care.
