The role requires a BSN or higher, an active RN license, and 1+ years in case management, with strong EHR proficiency and communication skills to support patients and families.

Qualifications

• BSN or higher from an accredited program
• Active and unencumbered RN license
• 1+ years experience in case management, preferably in healthcare or hospital setting
• Exceptional interpersonal and communication skills to collaborate with medical professionals, patients and families
• Proficiency in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive assessments of patients, including history, medications, treatment plans and psychosocial needs
• Collaborate with healthcare providers, patients and families to develop and implement individualized care plans
• Monitor and evaluate patient progress, adjust care plans as needed and inform the healthcare team of changes
• Serve as the main contact between the patient and healthcare providers to ensure seamless communication and care coordination
• Maintain up-to-date records and case notes in compliance with confidentiality and standards
